A pt has failed oral iron. And that is all that is documented. They have IDA and will be receiving Injectafer. Is this coded as:

D50.9 - IDA
T45.4X5S - Adverse effect of iron and its compounds sequela

or

D50.9 - IDA
K90.4 - Malabsorption due to intolerance NEC

Documentation only states failed oral iron.

This is not enough information. The physician should be queried. Documentation needs to state specifically how the patient failed oral iron.
