Is it 27091 even if the femoral stem is left in place? The femoral head, acetabulum, liner, and screws were all removed. A cement spacer was inserted on top of the stem. Do I need modifier 52 possibly?

The "Key" word here for me is "Complicated". What made this complicated to choose 27091 rather than 27090? If removing the components were routine, nothing complicating the procedure, I would use 27090.
