Wiki Excision of lesion vs. Tumor

I have a physician that documents excision of perioanal lesion. In the documentation of the procedure, he states it's a full thickness excision. He wants it coded using 45172. My problem is the path report states it was a skin tag. I would have preferred to code his as 46230, but because of the full thickness, management wants it coded the way the physician is documenting is. They prefer me to go by the op report and ignore the path report.

I'm not clinical, so I'm having a hard time with a full thickness excision of a tumor when the path said it was a skin tag. I though skin tags grew outwardly and could be snipped with a snare technique especially when it measured 0.6 cm.

Please advise: :confused:
