I'm having trouble figuring out how I should code this procedure. One of my doctors did an endoscopic approach for a reduction of a malar fracture. He also did a Caldwell-Luc approach and packed the sinus cavity with gauze soaked in polysporin to elevate and support the fracture. I cannot find a code that supports the work done by my doctor. He did the work of code 21385 (open treatment of orbital floor blowout fracture; transantral approach) which is the Caldwell-Luc type operation. Only the patient does not have an orbital floor blow-out fracture. Wondering if I should code 21360 (open treatment of depressed malar fracture, including zygomatic march and malar tripod) with code 31030 (sinusotomy, maxillary; radical without removal of antrochoanal polyps). Any help or suggestions would be much appreciated. This one has me stumped!
