Patient came in for a yearly routine physical exam and has chest pain a night before he/she came in. Can the doctor bill both preventive (ex. 99326) and office visit (ex. 99213) with modifier 25?
 
Hello,

Thanks very much for a quick reply. That's what the CPT guideline said. Actually there was a typo of the preventive medicine code. It should be 99381-99397. Do we need to have a two separate SOAP/progress notes?
