If the condition is no longer being treated but has the potential for recurrance, then the V code should be used. Also use the V code if the history of this illness would impact the current care being provided.
If there is documentation elsewhere in the record that indicates the hepatitis is still active and being treated, then use the 070.xx code if the provider addresses it or it impacts the care recieved during this encounter.
