My provider put incarcerated umbilical hernia with mesh for the diagnosis in his soap note, but coded this 553.1, 999.9 & V45.89. Wouldn't it be 552.1, and why would he code the other 2. This is my first time coding for a general surgeon, so why would he code the other 2? Am I missing something? I'm confused:confused:
