Can anyone explain how to code a DVT documented as happening on 1/14 (almost 3 weeks ago) but it doesn’t state whether it’s acute or chronic. I don’t see any guidelines in the coding Manuel regarding this either. Is there a certain time frame when a DVT is classified as chronic. The patient is documented to be on Xarelto as well. Should this code as acute or chronic? Is there a default code or would the provider need queries if available?
 
It is best to query your provider for clarification. However, assuming this is a lower extremity DVT for purposes of this answer, the notes under the code I82.40 in ICD-10 state DVT not specified as chronic or acute is coded as acute. Use of medication does not exclusively indicate a chronic condition.
