Wiki Does this meet MIPS #397 for Melanoma?

I have a report of a melanoma re-excision. The diagnosis includes "small focus of residual invasive malignant melanoma." So, this qualifies for MIPS #397 for Melanoma. The report includes all of the criteria except for pT. The report says at the end to refer to the original biopsy report for the pTMN staging.

I don't think that's sufficient for G9428, but would appreciate other input. Thanks!
 
I would say that is not met. Each report that documents melanoma needs to specify all the documentation requirements including pT category. I recently did our MIPS and had to place a couple reports under "not met" for that reason. :)
