Wiki Does excision of breast lesion now include localization?

With the new 2014 placement of breast localization device(s) codes 19128-19288, I get an NCCI edit stating that these codes are component of CPT 19125 Excision of breast lesion identified by preoperative placement of raiological marker, open; single lesion. Does anyone know why? Do the breast lesion excision codes now include placement of a localization device? If the localization device is placed in radiology prior to the patient going to surgery is a 59 modifier appropriate on the localization code? Hospital coding. Not sure what has changed in 2014 with CPT 19125. In 2013 there was no edit for the old pre-op localization code with 19125. Thanks!
 
I'm also looking for official coding guidelines on this. I've checked Coding Clinic for HCPCs and CPT assist with no luck. As of now I'm leaning to add modifier 59 for facility coding because we have two separate physicians doing the localization and biopsy.:confused:
