I am having some issues with billing out for L3000 and L3030. I am billing out with modifier KX. The claims are continuously denied stating that the modifier is not correct or a modifier is missing. The Noridian MAC site does state that this modifier is current and is to be used for orthotics. Does anyone have any suggestions? I feel pretty stuck.

Also, does anyone know of any manuals that give guidance for Medicare billing for orthotics?
