Is there a specific guideline that instructs us to use a bilateral diagnosis code of H25.813 instead of using the diagnosis codes H25.811 and H25.812? I know there is a guideline for glaucoma coding but have not been successful in finding instructions about cataract coding.
 
Yes, the guidance on coding of laterality can be found in the ICD-10-CM Official Guidelines, in Section I.B.13, which includes the instruction: "If no bilateral code is provided and the condition is bilateral, assign separate codes for both the left and right side."
