When billing for device monitoring (93268 - 93298, G2066), should the z-code be billed as primary or secondary (i.e. Z45.02, z45.018, etc)?
 
The provider's documented reason for the service in the medical record will determine what codes you use and what the sequence will be. The Z45 codes are typically used when the reason for the encounter or service is for scheduled routine monitoring or adjustment of a device when there is no patient condition or symptom that is under evaluation or active treatment at the that particular encounter.

This is routine monitoring and we typically use the same diagnosis as the reason for implant. but would that be primary or the Z code as primary

Where I've worked, we've coded the reason for the visit as primary (the Z code if it's routine or for maintenance of the device) and the underlying condition as a secondary code (such as sick sinus syndrome if that was the reason the pacemaker was placed, for example).
