Anybody else having issues with Medicaid denying add-on codes such as 11101 or 17003? They are paying the primary code but not the add-on. What is going on?! Thanks!
 
What state. Medicaid is administered at the state level and there may be different guidelines. Possible they just pay a flat rate regardless to the number of lesions.
