Wiki Denial of payment for cpt code 69990. Please help

My practice has recieved a denial from workmans comp. The patient was billed for 64831 primary and 69990 no modifers secondary. The denial we recieved states CCI; missuse of column 2 code with column 1 code. I do not see anywhere that these code are bundle. If anyone can help I'd appreciate it.:) Thanks, Maria
 
It might depend on what state you are coding for. I know for my CA WC I have to use 61712 instead of 69990. But for my Michigan site I am not to bill 69990, no carriers will pay.

the 69990 is actually an add on code and the 64831 is listed as a primary for it, I would print that off and submit it with an appeal.
