Just need some clarification. Do codes 52234, 53325, and 52240 always include biopsies if done?
 
Yes, these codes would include any biopsies at the same site(s) and same encounter as the main procedure itself - i.e. in this case any biopsies of the bladder performed in the same operative session would be included.
 
What about if you did a TURBT of a posterior wall bladder tumor and did a bladder dome biopsy? I thought you could bill separately for those? 52234 and 52204.59?
