I am getting denials for the following codes from Medicare: 33208(with modifier sc) 33225 and C2621. The DX are correct from what I can find on covered indications for cardiac resynchronization They are for bi-ventricular pacers. I am wondering if it's the modifier that triggers rejection / denial or something else I am missing?
