Critical care for neonat or ped both inpt. and outpt.

On page 23 in CPT prof 2015, at the bottom right hand colum is a section that describes how to code for critical care for neonatal or pediatric pts. So to begin CPT states that "if the same individual provides critical care services for a neonatal or pediatric pt. in both the outpatient and inpatient settings on the same day report only the appropriate neonatal of pediatric critical care code 99468-99472." CPT 2015.

O.K. so I go to the code description on page 44 for 99468 - 99472 and we have the 2 codes for initial and subsequent neonate critical care 99468 and 99469. Then there is the codes for infant 29 days through 24 months which are 99471 and 99472. This brings us to the end of the directions on page 23. There's no further directions for the next age group which is age 2 through 5 yrs. (see page 44).

This age group has CPT codes 99475 and 99476 available to assigned for critical care on pg. 44. How should the age group 2 through 5 yrs. given the scenario above since CPT isn't directing us to code for this group in this scenario? Thanks
