Hello. Looking for codes to use for the prior auth.
Patient currently has a dual chamber pacemaker.
Doctor will be removing the pacer and implanting a dual chamber ICD and will use the current atrial lead from the pacer. Will be adding a new ventricular lead.
The codes I came up with are: 33233 for pacer removal & 33249 for ICD insert with single or dual leads.

Thoughts, words of wisdom are appreciated.
