Can someone please help I need to know the requirements for billing 99401 Preventive medicine counseling and 99408 Alcohol/substance abuse and the documentation needed.

In Ohio, 99401 Preventive Medicine Counsel and 99408 Alcohol/substance abuse; are both a non covered service per Medicare fee schedule. Per Medicaid NCCI policy Chapter XI;
CPT codes 99408 and 99409 describe services which are similar to those described by HCPCS codes G0396 and G0397, but are "screening" services which are not covered under the Medicare program. Where CPT codes 99408 and 99409 are covered by State Medicaid programs.
