Wiki CPT 34714 graft or no graft?

Hi! I was wondering if you would code CPT 34714, open femoral access for cardiopulmonary bypass, in right groin incision for a heart transplant (CPT 33945) if no extension graft was needed to access the femoral artery? Is the prosthetic graft “if necessary“ or required? My surgeons do not always needs a graft. Thanks!
