I have a surgeon that did an arthroscopic complete synovectomy and a Mumford on the same shoulder. In the op note he states he did the synovectomy through a posterior and anterior portal. He then removed all instruments and then placed the shaver/burr into the subacromial space from the anterior portal. I know that CPT 29821 is bundled into 29824 but in this case since he removed all instruments and then performed the Mumford do you think it could be appropriate to append modifier 59?
