Wiki covid testing with rapid influenza A and B

I wanted to see if anyone has any insight on when to bill 87428 for the Covid test plus Influenza A and B, versus 87426 and 87804 (x2). Does there need to be multiple swabs or documented a certain way?
I have been searching everywhere and I cant find anything that differentiates the two.
 
At our facility, we are using a BinaxNow that is capable of running both tests simultaneously; however, we are not utilizing this feature for "unspecified CLIA reasons"

My understanding is that if you perform both tests simultaneously, use 87428-QW. If not simultaneous, use 87426 and 87804 (x2)
 
I'm trying to better understand the CLIA waived, the company I work for has a lab facility come in house and do draws their draws so on certain labs will they get certain modifiers? Like the QW
