OP NOTE READS: This was Laparoscopic
There was an indirect inguinal hernia which was reduced and the peritoneum was separated from the spermatic cord structures which were all identified and preserved. This was then carried down to the level of the psoas muscle. There was a cord lipoma in the inguinal canal which was also removed.

Is the cord lipoma billable?
 
I've always considered the excision of the cord lipoma to be an incidental procedure during a hernia repair, though I don't have a reference for this in the context of a laparoscopic hernia repair. The CPT code 55520 for 'excision of lesion of spermatic code' carries the 'separate procedure' designation, and while this is not a laparoscopic code, it indicates to me that this would always be incidental to any other procedure performed in that same session and through the same incision.
