Can you bill for coordination of care for patients with Chronic issues? I worked for pediatricians and we have a few patients that live in facilities and at home. My doctors would like to periodically bill for coordination of care for all the medical forms, home care plan forms, medical equipment, prescriptions, OT, PT etc. that they need to do on an on going basis.

I looked in the CPT Manual and the Complex Chronic Care Coordination Services code, but not sure if this really applies. Any suggestions? Not sure that this is really something we can bill for.
