If a requesting provider has access to the same chart as the consulting provider, and can read the progress note, is that enough to meet the "report" requirement?

Might be just splitting hairs, but if the note says: thank you for you "referral", would we call it a consult? I only ask because I have seen that exact phrase documented before when it was supposed to be a consult.

They were referred for a consult. If the components of a consult are there, then it's a consult.
