I work with a PCP and I have a question, (Doctors order) I have procedures done with a visit in which I coded 99215 -25, 11200 w' 701.9 skin tags, 17004 w' 702.11 infl. seborrheic keratosis and 14700 w' 239.2. and of course ins. didn't pay. I need help in what mod do I need to add. Thanks all for any input
 
I would need to see the note to be able to advise. I would question the visit level and the Dx codes. The 239.2 is a specifically concern. If the provider called it a skin lesion then you cannot use this code. A skin lesion is 709.9. 14700 is not a valid procedure code and 17004 will not be supported with the DX code you have indicated.
