Wiki Condition No Longer Exists

Is there a time frame in which a diagnosis should no longer be assigned to a condition? Example: Patient had a kidney stone in 2000, but nothing symptomatic since then. (He is being followed for CKD)
Should we still be coding the 592.0 as a condition or should it be changed to history-of (or not coded at all)?

Does it state anywhere in the chart that he still has the stone, rad reports and/or HPI??
Also, is it assessed or treated any type of way within that encounter, if that is the case I would stick with the 592.0

If not, I would do a hx of code. Since he is being followed for CKD and it falls within the same body system.

I would like to hear what others think as well :-)

The note only states that 'he hasn't had any epidodes of nephrolithiasis for several years' and 'no problems since 2000'
