Can someone tell me if a Complete PFSH is required for a Medicare NEW patient exam? I need to find it in writing citing whether it is or is not required.
 
Your best bet is to check your MAC to see what their E&M guidelines are. However, CMS' E&M Guidelines state:

You must document at least one specific item fromeach of the history areas for these categories ofE/M services:
™ Office or other outpatient services, new patient
™ Hospital observation services
™ Hospital inpatient services, initial care
™ Consultations
™ Comprehensive NF assessments
™ Domiciliary care, new patient
™ Home care, new patient
