I need to find a cpt code for a procedure done in office after a fistulotomy was done. They were coming back into office to have the seton tightened but rather then that, they dividing both tracks under local anesthesia

Divided the remainder of the fistula tract in office today under local cpt code

Here's my best shot

I'm a coding student, but here's my best shot:

46715-78, for Repair of low imperforate anus; with anoperineal fistula (cut-back procedure), though I'm not certain if the 'cut-back procedure' correlates with the division of the fistula tracts.

Modifier 78 being used because it seems to be an unplanned return to the operating room by the same physician/healthcare professional following an initial procedure for a related procedure during postop.

I also looked up Division in CPT, and found > Rectal Stricture > 45150 (Division of stricture of rectum), but I don't know if that could apply to the fistula tracts... If it does, perhaps 45150-78 would be worth considering?

Hope this helps!

Sincerely, Jacob
