Wiki Coding labile HTN with a definitive diagnosis of HTN

HI,

I have a provider who keeps documenting that the patient is hypertensive (and on meds) but patient is also experiencing white coat syndrome. Do I code both r03.0 and I10 and if so in what order (pt came in for BP med check).

'Hypertensive' is coded as I10 per the alphabetic index. R03.0 would not be supported by the documentation you've given here, and would also not be an appropriate code to use in conjunction with I10 because a high blood pressure reading is already a symptom of the hypertension diagnosis and shouldn't be coded if the definitive diagnosis has been made.
