When we code for a TPI 20553 and we use 3 cc of the Bupivacaine (code J0665) and 1 cc of the Triamcinolone (code J3301) how many units should we code the J0665 and J3301?
 
Only J3301 should be billed with 20553. Bupivacaine is included in the injection CPT code since it's a local anesthetic. In the beginning of the surgery section in CPT it says the below. The number of units for J3301 depends on the strength of the Kenalog, which needs to be documented. Usually it's 40 mg/mL, so 1 mL would be 4 units (J3301 = 10 mg).

The CPT codes that represent a readily identifiable surgical procedure thereby include, on a procedure-by-procedure basis, a variety of services. In defining the specific services "included" in a given CPT surgical code, the following services related to the surgery when furnished by the physician or other qualified health care professional who performs the surgery are included in addition to the operation per se:
Evaluation and Management (E/M) service(s) subsequent to the decision for surgery on the day before and/or day of surgery (including history and physical)
Local infiltration, metacarpal/metatarsal/digital block or topical anesthesia
