Patient was seen at PCP one month following hospital discharge for treatment of subarachnoid hemorrhage. Discharge DX was hydrocephalus 2/2 aneurysmal subarachnoid hemorrhage. PCP coded as I67.9 Cerebrovascular disease, unspecified. Patient is having headaches but provider only referenced in the HPI and did not link to the hemorrhage. Hemorrhage was not due to trauma.

I am an HCC coder and wondering if appropriate to code the sequelae (headache) following the hemorrhage or code as Z86.79 Personal History of other diseases of the circulatory system. From the documentation cerebrovascular disease is not supported.

Interested in hearing from the coding community on this issue.
