I have a chart where the patient came in with pregnancy symptoms- HA, fatigue, dizziness, food cravings, nausea, vomiting, lower abdominal pain, lower back pain. She also has not had a period since she gave birth, 1 year and 4 months ago. They ran a blood and urine preg test and patient is not pregnant. The provider selected O21.0- morning sickness in our EMR, which is incorrect. So I am coding the signs and symptoms. I coded all of the ones listed above, but I am unsure about the absent menses N91.2. Can I code this as a symptom or does the provider need to diagnose this as a condition for me to code it? Also should I add Z23.02- pregnancy test negative? Basically how should I code this? Thanks in advance!

If the provider documented an absence of menses for 16 months duration then yes you may code secondary amenorrhea along with the negative results
