Need insight on billing 69930 and L8614 for Medicare and commercial insurance
1. Can provider bill both the codes along with E/M?
2. If the procedure was done at a clinic which is part of the hospital would hospital bill both? What about E/M code with modifier 25? Will provider also bill for 69930 or only E/M?
3. Is 69930 and L8614 used only in hospital settings?
5. If the procedure was done at a clinic which is part of the hospital setting what are the billing guidelines?
