Do Emergency room physicians ever bill on a UB04 for an ER visit?

The physician bills on a CMS-1500.

(There might be some very specific circumstances for Critical Access Hospitals (CAH) where the facility bills for professional services too, but there are specific rules and written attestations involved. If you're billing in the Critical Access Hospital setting, you're going to want to get information specific to that. It's not my area of expertise, so I really can't advise on that subject.)

Edit to add: I'm guessing you may not be working with CAH hospitals, since I don't think New Jersey has any.
