I am receiving denials for code 90960 with one medicare blue requesting documentation for said services. It was paying consistently than on October it stopped with request of medical records. has there been a change or do i need to send all face to face visits out with a zero dollar to support the code on the same hcfa? and if so what code am i using

Hi. I saw an article today that medicare made millions of dollars in overpayments for capitation payments for this service, so perhaps they are now being extra cautious.
