My hand surgeon did a revascularization of the ulnar artery with cephalic vein graft.

I am having a hard time coding the graft.

"we harvested a branch of the cephalic vein measuring 3.5 cm in length. The vein graft was reversed. It was irrigated with heparin. Then placed in the palm of the hand using the operating microscope, the anastomosis of the ulnar artery to the vein graft was performed distally to the palmar digital artery and proximally to the main ulnar artery."

Any help would be great.
