how would you code for a dropped lens during a cataract surgery , please help

What do you mean by dropped lens? Was the IOL dropped on the floor or was the posterior capsule torn and the IOL went into the vitreous chamber?

I apologize I wasn't clear. During the surgery the posterior capsule torn and the IOL went into the vitreous chamber

Not exactly sure about this but here's at least a starting point.
Your ICD-10 would be T85.22xA for displaced IOL

CPT: 67036 (pars plana vitrectomy) + 67121-51-59 (removal of IOL from posterior chamber) + 66985-51-59 (secondary insertion of IOL)
