OK,

Help me settle the score - conflicting opinions on this.

During a reversal of a diverting ileostomy, the following appears in the op note:

The small bowel was dissected from itself and its attachments in the pelvis. There was one loop of small bowel that was torn during the dissection. It was resected and the bowel loops reanastomosed with GIA-75 stapler.

So, can I bill for the small bowel resection caused by risks of the surgery? What CPT and DX code would I use for this?

Any help is greatly appreciated.

Hunter Smith, CPC

The tear of the loop of the small bowel was incidental to the reversal of the ileostomy and is not separately reported, you would just bill for the ileostomy reversal and use Codes 997.4(Digestive system complications) and E870.0(Surgical operation) for the misadventure.
