Can the Interdisciplinary Team Conference code G0175 be billed by a physician/professional on the 1500 form? When I'm determining a rate for it, I can only find it on the OPPS fee schedule.

If it can be, how do I go about finding the appropriate rate?
If it can't be, what code(s) should be used instead of G0175?
