Surgeon performed a CABG and left ventricular aneurysm plication. Any ideas on a code for the aneurysm plication?


The veins were carefully oriented and sized and kept out of harms way. The left ventricular aneurysm was opened. The aneurysmal segment which was identified on ventriculogram and was significantly dyskinetic was entirely necrotic. Horizontal mattress pledgeted sutures of 2-0 Prolene were then placed in what appeared to be a viable ventricular muscle from the free anterior wall through the most anterior portion of the septum. Six sutures were required to entirely exclude the aneurysm. These were then reinforced with 3 more horizontal mattress pledged 2-0 Prolene sutures placed somewhat more distally in order to completely occlude the aneurysm and reestablish ventricular continuity.
