Can cpt 46285 and cpt 46288 be billed together or is it considered a "bundled" service.
 
46285 is incidental to 46288 so they cannot be billed together.

Unless these are two separate fistula tracts. Then you can bill a -59 modifier on 46285 as long as your documentation specifies two separate sites and two separate incisions.
