There is no code for "bilateral" hand joint pain. You would code specific to the documentation; if the provider documents the patient has joint pain in both hands, you code for each hand, M25.541 and M25.542. M25.549 would be used if the provider did not specify which hand the joint pain was in.
 
Thanks for your reply. Where I work providers do there own coding then we validate,provider documented BL hand joint pain, code M25.549 unspecified but imaging was ordered for R and L hand which makes it specific. i felt like M25.541 with M25.542 would be the correct coding.
