I have a question in regards to billing the 0191T, insertion drainage used for cataract procedure 66984, I am a Harvard Pilgrim biller and they deny this code as a non-covered service. Does anyone have any information on how to get this paid, I can't seem to find out if it is dx driven, the dx I have seen providers using is the H40.51 ocular hypertension. Policy states in med policy it is exclusively used for Medicaid agencies, or is this just a case of appeal with LMON, med notes.. I would appreciate any information. :p
