Just started billing for loop recorder insertion
UB04 form CPT 33285 TC and C1764
I'm trying to figure out what the correct revenue code is for CPT 33285 TC I used 510 but claim denied
I used revenue code 278 for CPT C1764
I'm lost as to where to go with this ,would appreciate advise
 
A loop recorder insertion is a surgical procedure usually done in the OR, so I've always seen this procedure billed with revenue code 360. If it's being done in a procedure room in your facility, you may want to consider revenue code 760 or 761. Revenue code assignments can vary from payer to payer though, so you may need to consult your individual payer requirements for guidance on this.
