It has been brought to my attention that you can bill for BCN Health Qualification form for reimbursement when a patient has their annual physical. Does anyone know what the cpt code would be? The closest I have found is 99080. I have not found this code on the BCBS/BCN fee schedule.
Any assistance is appreciated!
