Having trouble figuring out which codes to use to bill flu vaccination.

There's no two-word answer here. First, you have to know what formlation of vaccine you're using and how old the patient is. Is it purchased? State-supplied? Who's your payer? How was it administered? injection, nasal, other? Was the patient a kid, so that there was additional counseling given (and documented) to the parent? Were other vaccines given at the same time?

Basically, you have a code for the vaccine (90654-90668) and the administration of the vaccine, 90460-90474. Medicare requires you use G0008 for the flu administration.

If you have more information, I could probably give you the exact codes.

Documentation

I have always been of the understanding that when billing for flu vaccines that a doctor must sign off on the chart in order to bill. We are having some conflict in our office with physicians not wanting to sing off on a flu shot given by a nurse. Any Help would be appreciated.

For the vaccine, you need to find out what flu vaccine was purchased and use the appropriate code for Medicare, with a G0008 for administration. For private insurances we usually bill 90658/90471. If its just a flu shot visit, we do not charge the 99211.
