Do Advance Care Planning CPT codes 99497 and 99498 only pertain to counseling and discussion regarding Advance Directives, or does counseling and discussion regarding Goals of Care count?
 
As an auditor, I can tell you that this code is high risk and billed without the necessary supporting documentation often. Look up the documentation requirements provided by the local MAC as well as payer guidelines if available.
