billing 20610 and 76881 when there is no "permanent record" documented for 20611

If your provider is trying to bill 20611 and there is no "permanent record or report" within the note and not found in the EMR, can you then bill a 20610 and a 76881? All of the documentation to support both of these codes are within the note.

You may want to do a bit more research with your clinic. I didn't think that our clinic was keeping the permanent record either, but I found out that we were. It was not getting to the chart as fast as you would like, but we did have them and they were uploaded. May want to see if the same thing is happening in your clinic too.
